Nurse Led Clinics

  • Coronary Heart Disease – If you have been advised by the surgery to complete an annual heart disease review, please submit an Annual Coronary Heart Disease Review form
  • Diabetes – If you have been advised by the practice to complete a diabetes review, please submit a Diabetes Review form
  • COPD – If you have been advised by the surgery to submit a COPD assessment, please use the COPD Assessment form
  • Asthma – please use our Asthma Triage
  • Smoking Cessation – If you have been advised by the surgery to submit smoking reviews on a regular basis, please use the Smoking Review form
  • Travel Clinic – If you are travelling abroad, please complete a Travel Risk Assessment to arrange any vaccinations that may be necessary

Sexual Health

If you have been advised by the surgery to submit a contraceptive pill review, please use the Contraceptive Pill Review form.

Dr A Loader will be holding a sexual health clinic on a Wednesday afternoon between 13:30 and 17:00.

The clinic includes a confidential sexual health screening service, emergency contraception and fitting and removal of implants and coils.

These are to be pre-booked appointments only. Please speak to a member of the reception team to book your appointment. These clinics will also be open to patients who are not registered at the practice and are part of a wider network of clinics being made available throughout Gloucester.

Please note if you need to have a contraceptive implant fitted or changed then a Doctor will need to prescribe this for you before your appointment so that you can collect it and bring with you to your appointment.

Children’s Immunisations

The practice nurses hold a children’s immunisation clinic on a Tuesday afternoon between 13:30 and 14:30 and a Wednesday morning between 08:30 and 11:00. If this is not convenient you can book a routine nurses appointment.

Maternity Care

Antenatal clinics are run in the practice by our midwifes.

Clinics are held on a Wednesday afternoon (by appointment only). For your first appointment with the midwife please use the NHS booking system.

Initially an appointment needs to be made with the midwife to collect your hand-held notes. A booking appointment will then be arranged, this may be a home visit at approximately 10 weeks gestation, to complete the paperwork.
